YOUTH INTELLECTUAL DISABILITY SUPPORT COORDINATION assists children with intellectual disabilities and their families in accessing needed psychiatric, medical, social, educational, vocational and other supports essential for health, safety and independent living. Referrals for youth with intellectual or developmental disabilities can be made by contacting the Developmental Disabilities Coordinator.
Support Coordination Services Include:

Individual identification and outreach

Comprehensive assessment to determine needed supports

Linkages with services and supports

Monitoring of service delivery

Assisting the individual directly for the purpose of developing or obtaining needed resources

Enhancing community integration through increased opportunities for community access and involvement

Parent support

Advocacy
-
PAL services- up to 5 hours per month

Therapeutic horseback riding once a month at Copper Crest Farms
