Adult
Case Management is a program which ensures that service systems
and community supports are responsive to the specific, multiple
and changing needs of individuals with Mental Retardation or those
who have a serious and persistent Mental Health Illness. Case Management
services make certain that individuals and their families are properly
linked to, and involved in, the appropriate services necessary to
live successfully in their communities.
Case
managers meet with people to assess their strengths, needs, and
goals. They work with individuals to develop a strategy, called
an Individual Service Plan, designed to meet those needs. Case managers
assist individuals in gaining access to things such as housing,
jobs, residential and day support services, social service benefits
and entitlements, social and educational resources, and psychiatric
and medical services essential to meeting basic needs and achieving
their goals. The individual service plans are reviewed regularly
with the individual and service providers to monitor progress, and
ensure the individual's satisfaction with services. Case managers
assist individuals in making informed decisions about treatment
and services, and also help them to understand their rights and
options.
