· INFANT & TODDLER SERVICES is a program serving infants and toddlers (birth through 3 years) who have (1) a 25% delay in one or more of the following areas; cognitive, physical (including fine motor, gross motor, vision and hearing), communication, social/emotional, or adaptive development, (2) atypical development or (3) a diagnosed physical or mental condition that has high probability of resulting in developmental delay. Referrals are received by the program coordinator and several direct service providers from parents, physicians, specialty clinics and various community agencies. A simple screening by a home visitor and/or professional evaluation can tell if the child has a developmental delay. If the child is eligible for services, an individualized family service plan is developed to meet the child's and family's identified concerns and priorities. Services to children and their families might include: (1) assisting family members with activities to enhance the child's development, (2) locating and securing needed resources for the child and/or family members, (3) coordinating and monitoring the services that are provided and (4) facilitating the development of a transition plan to preschool services, if appropriate. Referrals may be made by contacting (276) 223-3274 or (276) 223-3270.
