Our mission at Transitions is to provide quality assessment,
evaluation, and therapeutic services for individuals and families
trapped in the disease of addiction.
To help clients and their significant others find a deeper
sense of worth and self-respect---as well as develop a renewed sense
of confidence, courage, and hope.
To support individuals and encourage them to become more
productive citizens and to lead a more fulfilling life according to
their needs.
Program
Description and Goals
Transitions--located at 115 North Church Street in Marion,
Virginia---was established to offer services to clients who are
identified as having substance abuse problems that may be exhibited
by difficulty in maintaining relationships, employment, and health.Transitions provides a therapeutic, supportive environment
for clients seeking guidance in chemical-free lifestyles.
Specific services offered to individuals age 18 and over who
are voluntarily seeking treatment include:an Intensive Outpatient Treatment
Program, a Women’s Recovery Program, A Life Enhancement Program,
and Support Services.Services
to pregnant women are provided with coordination of prenatal care.Suicidal or violent clients are not appropriate and are
referred for further evaluation of treatment options.
Clients are discharged from the Transitions Programs upon
successful completion of treatment.The treatment team will evaluate other situations that may
result in discharge.These
situations may include lack of
motivation, lack of participation in activities and meetings,
relapse, or failure to comply with any/all rules of conduct or
regulations.Clients
may discontinue treatment against staff’s advice if he/she
chooses.However,
clients are encouraged to continue his/her substance abuse treatment
and referrals are made based on individual need.
Intensive
Outpatient Treatment Program
The goal of the Intensive Outpatient Treatment Program is to
provide a safe, supportive environment for the individual to explore
his or her issues with addiction.The focus is on education, motivation enhancement, and
exposure to Alcoholics Anonymous/Narcotics Anonymous.These activities are accomplished through participation in
alcoholism/drug addiction educational sessions, group therapy, and
Alcoholics Anonymous/Narcotics Anonymous meetings.Clients may enter the program at any time.After completing four weeks in the Program, clients will have
attended a complete sequence of lectures and groups.
During the Program, clients will be expected to complete the
following:
Participate in completion of ASI
assessment
Attend all
groups/meetings
Show
appropriate motivation and participation
Attend
goal-setting meetings
Maintain
continued abstinence
Select a
temporary sponsor
Complete
First Step self assessment
Assess
aftercare needs with SA Placement Specialist
Develop an
aftercare plan
Attend
Family Day prior to discharge
Any
relapse during the program will result in a treatment team meeting
and/or a referral for medical detoxification.The Counseling Coordinator or designee will schedule the
treatment team meeting to evaluate circumstances surrounding the
relapse and re-assess treatment needs.
Transitions is committed to providing a holistic,
reality-oriented approach to treatment for chemically dependent
individuals and their families.Transitions will provide an opportunity for people to
experience a new creative way of living without chemicals in a safe,
supportive, and challenging atmosphere.
Women’s
Recovery Program
The goal of the Women’s Recovery Program is to create a
safe and nurturing environment based on respect, mutuality, and
compassion.The Program
reflects a model that integrates the theory of addiction, the theory
of women’s psychological development, and the theory of trauma.The focus is on self, relationships, sexuality, and
spirituality.
Life
Enhancement
The goal of the Life Enhancement Program is to focus on areas
such as parenting skills, goal setting, anger management, and time
management.The program provides a safe, supportive environment in which
clients are able to reinforce skills already learned and replace old
behaviors with positive responses.
Supportive
Services
Supportive services are provided to clients who require
assistance in maximizing family and community resources.Clients receive training and education to promote a
chemical-free lifestyle.Staff
actively support clients in seeking employment, obtaining stable
housing, and providing transportation. Any additional problems the
clients may face are incorporated into the treatment process.

and Role of Staff
Transitions
is supported by the Specialty Services Director, Administrative
Coordinator, Counseling Coordinator, Office Assistant, Substance
Abuse Counselor, Substance Abuse Placement Specialist, Substance
Abuse Support Specialist, and a pool of Substance Abuse Community
Support Aides.Other
agency staff members are consulted on an as-needed basis.
